وانکوماسین
In all pediatric patients, when the organism is unknown, initial treatment is vancomycin 10 mg/kg IV every 6 hours or clindamycin 10 mg/kg IV every 6 hours and cefotaxime 50 mg/kg.
If suspicion of gram-positive infection, add IV vancomycin (15 mg/kg).
Add vancomycin 15 mg/kg IV (maxi- mum dose 2.25 g) when MRSA infection is suspected.
