Hyponatremia.
Hyponatremia is observed in 30% to 40% of patients with SAH, and is most commonly the result of cerebral salt-wasting syndrome (CSWS) or the syn- drome of inappropriate secretion of antidiuretic hormone (SIADH).193 Because these two conditions have different pathophysiology, correct diagnosis and management of hyponatremia is important, particularly when it precedes development of DCI and can worsen clinical outcome.
In CSWS, sodium is actively excreted from the body in high concentration, and accompanying diuresis occurs, despite ongoing hyponatremia with hypovolemia.194 Manage- ment of CSWS is the replacement of sodium with isotonic or hypertonic saline to restore volume depletion.
