Hysterectomy with Ovarian Conservation Tied to CV Risks - Risk particularly pronounced for women 35 and younger
Women who underwent a hysterectomy with ovarian conservation may be at greater risk for several cardiometabolic conditions, researchers reported.
Hysterectomy at any age was associated with modestly increased risk of newly diagnosed hyperlipidemia (HR 1.14, 95% CI 1.05-1.25, P=0.002) and hypertension (HR 1.13, 95% CI 1.03-1.25, P=0.01) in an adjusted model, according to Shannon K.
Also, women who underwent hysterectomy at age =35 years had a 4.6-fold increased risk of congestive heart failure and a 2.5-fold increased risk of coronary artery disease, the authors noted.
"Studies have shown that bilateral oophorectomy increases mortality and the risk of cardiovascular disease and other chronic diseases; thus, bilateral oophorectomy rates at the time of hysterectomy have decreased," wrote the authors.،Placenta accreta is the most common indication for a cesar- ean hysterectomy.
The risk that cesarean hysterectomy will be required increases with the number of previous cesarean deliveries.
A small focal placenta accreta can sometimes be excised and oversutured, allowing uterine sparing; however, in the majority of cases, cesarean hysterectomy is warranted.
As noted earlier, there has been some success with uterine preservation in selected cases; however, the majority of women still undergo cesarean hysterectomy.
